What should I expect after requesting an appointment?
1. An e-mail that your appointment has been confirmed or a declined (as that time slot/day is no longer available).
2. An e-mail requesting that you sign consent forms and provide your insurance information (admission paperwork must be completed 48 HOURS prior to appointment or appointment will be CANCELLED.)
